Welcome. Here is a guide to your intake packet. Please complete the forms and bring them with you to your appointment. I know these forms take time and effort on your part and I thank you for completing them.
1. Please read the packet containing the Consent to Treat, Personal Disclosure, and Notice of Privacy forms and sign page 9. These pages provide important information regarding my policies and procedures about sessions, confidentiality, fees, etc.
Please bring the signature page (page 9) with you. This form must be signed before your session can begin.
2. The fee agreement form will be placed in your file, it explains financial policies.
